Conditions We Treat -- Referral Guide
Knee Pain -- Genicular Artery Embolization (GAE)
Appropriate referrals from orthopedics, primary care, sports medicine, and rheumatology. GAE is indicated for moderate-to-severe knee pain from confirmed osteoarthritis after conservative care falls short.
Peripheral Artery Disease -- Catheter-Based Revascularization
Appropriate referrals from primary care, cardiology, podiatry, endocrinology, and wound care. Dr. Chong performs PTA, stenting, and atherectomy for symptomatic PAD, including claudication, rest pain, and non-healing lower extremity wounds. Urgent consultation is available for critical limb-threatening ischemia. ABI testing is available in office.
Dialysis Access -- Outpatient Access Evaluation
Appropriate referrals from nephrology, dialysis centers, vascular access teams, and primary care for fistula or graft concerns, difficult access, access maintenance, and related vascular evaluation.
Venous Disease -- Varicose Veins and Chronic Venous Insufficiency
We evaluate and treat symptomatic varicose veins, chronic venous insufficiency, and venous ulcers with in-office venous duplex ultrasound.

- Consultation scheduling typically within one to two weeks of referral
- Thorough pre-procedure evaluation and individualized treatment planning
- Consultation summary returned to referring provider within 48 hours
- Post-procedure report provided after treatment
- Direct physician-to-physician communication available for complex cases
- All procedures performed outpatient at the Jacksonville or St. Augustine office
About Dr. Kok Chong
Dr. Kok L. Chong is double-boarded in Diagnostic Radiology and Interventional Radiology and holds a Certificate of Added Qualifications (CAQ) in Interventional Radiology from the American Board of Radiology.
He completed his Vascular and Interventional Radiology fellowship at Indiana University Medical Center and has been in full-time outpatient interventional practice since 2011.
